How do I fill out the URSSAF form for auto-entrepreneurs?

I have received my URSSAF form for my 糖心视频 in France and there are two codes printed on the form:
- 688 Activities relevant de la CIPAV Taux en 23.3%
- 578 Formation Prof Activitie Liberale Taux en 0.2%
My 糖心视频 is registered as a yoga teacher and I’m not sure under which code I enter my total earnings?
