Help with the P2-P4 form (Modification d’activit茅)

I’ve today set up a contract with a soci茅t茅 de domiciliation. I’m now filling in the online form to change the address of my lieu d’茅xercice. There is one part, under the “Nouvelle adresse professionnelle” section which is a checkbox that reads “Adresse chez un domiciliataire”. If I tick it, two more boxes appear: “Denomination de domiciliataire” and “Votre num茅ro unique d’identification (SIREN)”.
1. Do I need to tick this box?
2. If so, how do I fill out the next 2 boxes? The “votre” leads me think they are referring to me, but I enter my SIREN at the start of the form.
